Transition & Prevention


The Administration for Community Living in 2015 recognized ‘Transition’ as a fifth core service. Transition can represent assisting people with disabilities in transitioning out of nursing homes or other institutional settings into a community living option of their choosing or it can be assisting youth with the transition to adulthood. SCIL provides both of these transition services to consumers in our service area.

SCIL has been assisting consumers with disabilities in locating housing of their choice since our very beginnings 30 years ago. SCIL’s Transition into Community Living program supports consumers in leaving nursing homes, rehab centers, and institutions to going back to community living with the supports they need in place. SCIL has been providing housing search assistance to Money Follows the Person (MFP) Demonstration participants in the Southeast region since 2013 with the MFP Demonstration ending SCIL will continue assisting consumers in transitioning into their choice of housing. We additionally provide housing assistance through our monthly Housing Workshops, if a consumer is unable to attend we offer one on one visits convenient for the consumer.

SCIL provides assistance to youth transitioning to adulthood through three different programs: Transition to Adulthood Program (TAP), Transition Internship Program (TIP), and Pre-employment Transition Services (Pre ETS).
